Outpatient Mental Health Services

Outpatient Mental Health Services are designed to serve consumers, children and families who require a less intensive level of care. Individuals reside in their home communities and meet regularly with either Master’s level clinicians or psychiatric/psychological practitioners for therapy, medication management, or referral to various other treatment programs. Outpatient services are appropriate for ages three and older.

Intensive Case Management / Resource Coordination

Intensive Case Management Services
Intensive Case Management (ICM) offers comprehensive coordination of care, service system linkages and oversight for the consumers enrolled within the program. Consumers who are enrolled in this program must meet strict eligibility criteria which include being diagnosed with persistent mental illness. The service is a supportive service in which workers engage in many activities designed to assist individuals living in community settings. This includes accompanying clients to appointments, helping clients manage day-to-day activities such as paying bills and shopping, and serving as an advocate for the clients should they require an inpatient admission to a hospital.

Resource Coordination
Resource Coordination (RC) is provided to adults in Warren and Forest counties with serious and persistent mental illness or emotional disturbances. Resource Coordination is a less intensive service than Intensive Case Management and its focus is more on the assessment of the consumer’s need and coordination of resources and services for the consumer. Staff in this program meet with their clients on a far less frequent basis than staff in the ICM program.

What is Peer Support?
Peer support services are structured activities for adults age 18 and older diagnosed with mental illness, provided by a Certified Peer Specialist. The program is an individualized, recovery-focused service that allows individuals the opportunity to learn to manage their own recovery and advocacy process from others living with mental illness who demonstrate ongoing success in their recovery.

Who is Peer Support for?
Peer support is intended for those presenting with mental illness that require, and are likely to respond to, therapeutic intervention. Individuals who have the capacity for participation in an individualized plan of care directed towards improving life skills and who are receptive to services in an unstructured environment without professional presence are appropriate.

Peer Support may be appropriate for individuals experiencing difficulties in the following areas:
- Is receiving or has recently received crisis intervention services
- Is experiencing functional problems in the home, community, church, school, job or volunteer activity.
- Is experiencing money management problems as a result of their ongoing mental illness.
- Would benefit from self advocacy skills to decrease dependency on more formal mental health systems.
- Would benefit from peer modeling in order to take increased responsibility for his/her own recovery.
- Needs peer supports in order to maintain a routine of daily living skills.
- Service prescriptions average between 2.5 to 5 hours weekly.
